A serious case review found a probe into the three-and-a-half-year-old’s injuries were “problematic” with “delays and insufficient follow-up”, despite the child and younger sibling being under protection plans because of suspected neglectful parenting.
Shropshire’s Safeguarding Children’s Board highlighted the findings from the November 2015 investigation in its latest report, due to be discussed at a Shropshire Council health and wellbeing board meeting today.
