Scheme that supports patients after they leave hospital to be scrapped
 

A scheme that supports patients who have been discharged from hospital will be scrapped to save a council £46,000 a year.
The "Hospital 2 Home" support scheme, which covers King's Mill Hospital, Mansfield Community and Ashfield Health Village, involves volunteers ensuring people are safe in their homes once they leave hospital.
But, according to Nottinghamshire County Council, usage of the scheme has been declining since 2014, and the current low numbers of people mean the scheme has hourly costs of £119.
