Extra Energy botched my switch and nine months later still won't send me a bill - blaming 'technical problems'

Last October I switched my energy supply to Extra Energy but cancelled the switch three days later, as I realised I couldn't give them a meter reading when required as I would be out of the country.
I then came back from my trip and in mid-December decided it was time to switch but then discovered my gas supply had actually been switched to Extra Energy two months earlier, without my knowledge.
It then took a few weeks and several phone calls to move my electricity onto Extra Energy as well and although I requested a bill, I'm yet to get one – almost a year later.
