Crossrail firms fined £1million after worker crushed to death on site
 

Companies working on London’s Crossrail have been fined more than £1million after a series of health and safety failures which led to one worker losing his life.
Rene Tkacik, a 44-year-old worker, was killed when a tonne of wet concrete was accidentally poured on him in 2014.
Another worker had his leg crushed by a tipper truck, while a third was injured after being struck by high pressure mixture.
The three accidents have resulted in the companies being taken to court by the Health and Safety Executive (HSE).
